Colliding with his teammate Finn Russell, both players looked incredibly groggy with Graham coming away from it worse off.
One fan summed it up perfectly, writing on X: “Fingers crossed for Darcy Graham. Grim collision.”
After treatment, the winger was put in a neck brace and placed on a stretcher before being taken off the pitch.
As for Russell, he underwent a HIA (a head injury assessment) before it was decided that he too would be replaced.
Scotland head coach Gregor Townsend gave an update on the pair, confirming Graham was taken to hospital, but is optimistic they’ll both recover well.
“Finn is fine,” Townsend said after the match.
“He actually passed his HIA but our staff sensed he wasn’t totally aware of what was going on, so we made the call to not put him back on.
“Darcy is at hospital now, I’ve not had an update. I think everything was OK with his neck but they were checking out concussion.”
With just two backs on the bench, Graham was replaced by scrum-half Jamie Dobie, while Tom Jordan, an inside-centre, replaced Russell at fly-half.
Scotland were already 7-0 down when the accidental collision between the two teammates took place.

However, things got much worse for the Scots.
Ireland pressed forward and went in 17-5 up at half time, before going on to win the game 32-18.
Sam Prendergast was the star of the show for Andy Farrell’s side, successfully landing three conversions and two penalties.
Speaking after the game, he said: “It was an unbelievable game and a savage atmosphere. The anthems were pretty special.
“I thought we started unbelievably in the game. We put so much pressure on. We did really well in the second half when they got their purple patch to come back.
“It wasn’t perfect from us but it was better than last week and we’ll keep trying to get better week on week.”
Scotland did enjoy 55 per cent possession during the match, while Ireland were forced into making over 70 more tackles during the game than their hosts.
However, stats don’t tell you everything with Ireland dominant throughout and Scotland being completely outplayed.
It means that Ireland now top the Six Nations table with two wins from two.
They have ten points so far, including two bonus points.
Meanwhile Scotland sit fourth in the table with one win and one loss to their name.
Just one point behind England, it means that the Calcutta Cup clash on February 22 may just have a bit of extra spice to it.
